  • We are looking for a compassionate, caring & experienced Registered Nurse to join our amazing care team as Care Manager at our beautiful long term care facility, Sun Rivers in Kamloops, BC.

    Location: 580 Shuswap Road, Kamloops BC

    Salary: $107,000-115,000

    Responsibilities

    • Oversees the provision of clinical care and provides supervision, leadership and guidance to designated staff and team professionals to achieve excellence in the care of residents and families

    • Works in collaboration with the client’s site management team, interdisciplinary team, residents/ families and stakeholders to identify and alleviate barriers to effective provision of care.

    • Coordinates work activities to ensure an efficient and high standard of care and effective utilization of resources to meet operational needs.

    • Provides support and supervision to designated staff and team professionals including employing effective communication strategies, conflict resolution, performance recognition, performance management and safe work practices.

    • Works collaboratively with staffing coordinators, or designates, to identify and resolve day-to-day staffing issues to meet staffing requirements.

    Qualifications

    • Current RN registration with the British Columbia College of Nurses & Midwives (BCCN&M)

    • At least two (2) years’ recent clinical experience working with older adults and in a residential care environment.

    • One (1) year of progressive leadership experience in a supervisory and/or administrative capacity in residential care setting or an equivalent combination of education, training and experience.

    Responsibilities

    • Develops, implements, and evaluates recreation/leisure programs such as crafts, music, mental aerobics, exercises, and sensory stimulation to meet residents’ physical, social, emotional, intellectual, and spiritual needs and interests; develops the goals and objectives of the program.

    • Provides supervision to all recreation staff, students, and volunteers.

    • Monitors, evaluates, and documents resident progress within the recreation/leisure programs and reports observations such as emotional wellbeing and social behaviour to the multidisciplinary team as required. Participates in multidisciplinary team meetings and care conferences. Identifies and makes recommendations regarding community and facility resources that may be of additional interest or need to the resident.

    • Monitors program expenditures and provides input to the program budget; oversees inventory of equipment, tools and materials and recommends purchase of same. Purchases minor equipment in accordance with established facility guidelines and procedures. Ensures equipment, tools and materials are in good repair and appropriate for use.     Responsibilities

    • Organizing and conducting site tours for all potential residents and/or interested community members or groups.

    • Coordinating the pre-admission and admission processes, timing and preparation, meeting the new resident and family as they arrive and escorting them to their new room.

    • Preparing and routinely updating resident social histories as needed

    • Using accurate, effective and timely communication with residents, families and visitors, staff and community partners while ensuring confidentiality guidelines are followed as they relate to residents and staff.

    • Providing consultation, education and support to residents and their families directing them towards appropriate government and community resources

  • We are looking for a confident, caring, and experienced Clinical Aide Lead to join our amazing care team at Sun Rivers in Kamloops, BC

    Responsibilities

    • Reviews 24-hour report and identifies residents with significant decline i.e., palliation, wounds, medication changes, behaviours, assessments and required follow up action planning 

    • Works alongside Nursing Team Leaders to identify plans of action, facilitates clear communication with all disciplines and ensures adequate follow-up. 

    • Completes daily rounds, providing clinical presence to each neighbourhood of the home 2-3 times per shift. 

    • Be present in dining areas during resident mealtimes to provide supervision and support.

    Hourly Rate: $55-$65 based on experience.

    Responsibilities

    • Adhering to the Ministry of Health’s Adult Care Regulations and legislation.

    • Developing, evaluating, monitoring and completing nutritional assessments and nutritional care plans for all residents upon admission and as required thereafter.

    • Organizing and maintaining the nutrition assessment and review schedule with current resident list.

    • Updating the resident’s diet binder for food service employees.

    • Identifying residents at nutritional risk and prioritizing accordingly.

    • Planning and monitoring therapeutic diets, dietary restrictions, and supplemental feedings to meet the specialized needs of residents.

    • Monitoring monthly weights and reviewing all significant weight changes.

    • Planning and reviewing menus every six months as well as the corresponding therapeutic and texture modification diets and implementing food production controls.
